Investigation Summary
At approximately 10.30 a.m. on August 23, 2000, Employee #1 went to pick up some merchandise. Employee #1 parked the pickup truck with its bed facing towards the door of warehouse and an employee at the warehouse came with a pallet with a few boxes on a forklift and tried to put it on the bed of the truck. The pallet would not fit. Employee #1 then climbed on the bed of truck and tried to make some room. Employee #1 stepped onto the pallet that was resting on the forks of the forklift. The pallet shifted and fell. Employee #1 fell approximately 40 in. with the pallet. Employee #1 suffered a compound fracture to his left elbow and was hospitalized.
