Investigation Summary

Investigation Nr: 201792660
Event: 08/19/1999
Employee Finger Injected with Gel Requiring Amputation

At 10:30 p.m. on August 19, 1999, Employee #1 was working in the gas room, on line two, where bottles and cans were being injected with gas and filled with shaving-gel product for Gillette. During the process, one of the bottles or cans burst and foamed. Employee #1 removed the bottle and checked and cleaned the gas injector with his finger without stopping the machine. The middle finger of Employee #1's left hand got caught and was injected with shaving gel. The finger later developed an infection and was amputated at mid-nail level.
