Investigation Summary
At approximately 10:00 a.m. on November 4, 1999, Employee #1 was using a modified polishing machine equipped with a mill-cutting disk to cut off the weld flash on motorcycle rims. The weld flash is a raised area of weld on the inside and outside of the rim where the rim is welded together. Once the weld flash is removed a belt sander is used to completely remove the weld. A coworker set the machine up and trained Employee #1 on how the remove the weld flash. Employee #1 had removed the weld flash on approximately fifteen rims when the accident occurred. Employee #1 held the rim in his hands and pressed the rim against the horizontal polisher with the mill cutting disk. Employee #1's right hand fingers were pressing up on the bottom side of the rim and his left hand was holding and guiding the rim. The only thing keeping Employee #1's hands of out of the dangerous point of operation was the rim. The work gloves that Employee #1 was wearing became entangled in the teeth of the mill cutting disk causing his right hand to be drawn into the point of operation. The coworker was preparing to polish some rims and was working about 10 ft behind Employee #1 when the accident occurred. The coworker said he heard the rim drop, and then Employee #1 ran over to him holding his right hand. The coworker noticed that Employee #1's glove and fingers were still entangled in the mill cutting disk of the modified polisher. Employee #1 was hospitalized for injuries that included: damaged tendons on the back of the right hand and right middle finger, amputation of the right ring finger and little finger. Employee #1 was hospitalized.
