Investigation Summary

At approximately 8:00 a.m. on August 5, 1999, Employee #1, of the City of Glendale, was removing a basket that had fallen into the hopper at the back of trash collection truck #7-238. His left hand was on the flipper near a bar hook when a coworker standing about 5 ft away activated the flipper control, catching Employee #1's left ring finger in the pinch point. He suffered a fractured and lacerated finger. The coworker later stated that he activated the control to lower the flipper and to make it easier for Employee #1 to remove the basket. He was unaware that Employee #1's hand was so near the pinch point.
