Investigation Summary
At approximately 11:30 a.m., Employee #1, an electric contractor, was in the process of lifting a steel plate measuring 4-ft by 8-ft by 1-in. by using a hoist steel chain and a Bobcat. He lifted the plate 1 ft above the ground and tried to position it to cover a trench 20 inches wide by 36 inches deep. His right pinkie finger got caught between the chain and the O-ring amputating it. He was hospitalized.
