Investigation Summary
At approximately 4:45 a.m. on November 15, 200, Employee #1, along with two other workers , went to the school site to reclaim classroom recyclable materials. The employees drove early in the morning to the site so that he could do the job before the school children would start arriving at the school site. At the school site, they found that recyclable material was mixed with other non-recyclable waste. They started to segregate the recyclable material from the non-recyclable. While doing this, the maintenance manager came to find out what was going on. The employee informed the maintenance manager that they were segregating the waste from the recyclable material and asked him to use the roll-off container for disposal of waste. After the maintenance manager left, Employee #1 walked around the truck and attempted to get into the operators seat. The employee did not remember why he was going to move the truck. Suddenly the truck started to roll forward, pinning him sideways between the lifting arm of the truck and the roll-off container next to the truck. The employee suffered two broken ribs, two broken neck vertebrae, a punctured lung and other internal injuries. He was transported to a hospital where he was admitted and released two weeks later.
