Investigation Summary

Investigation Nr: 201118478
Event: 09/29/2004
Employee Is Injured in Fall from Loading Ramp

On September 29, 2004, Employee #1, with Foster Farm Dairy, was unloading milk from his trailer and moving it into an empty trailer. He parked his trailer with its side door at the back of the empty trailer. Employee #1 then opened his side door and placed a dock ramp from the side of his truck to the back of the empty trailer. He was not able to place the dog ears on the right side of the truck due to crates of milk that had been loaded too close to the side door. Employee #1 climbed onto the ramp to push the milk crates out of the way. The dock ramp then slipped out from underneath him, causing him to fall approximately 51 in. to the ground. Employee #1 was hospitalized for serious injuries to both ankles. The cause of the accident was failure to secure the ramp to prevent it from slipping.
