Investigation Summary
At 9:30 a.m. on July 8, 1999, Employee #1 attempted to remove an obstructed cornhusk from underneath a machine that became clogged at the Tadin Herbal and Tea Company. Employee #1 turned off the machine, but the blades of the machine remained in motion. Employee #1 introduced his finger into blade area. Employee #1 sustained a scrape injury to the right index finger, and was transported to Kaiser Permanente Hospital where he was treated.
