Investigation Summary
At approximately 8:10 a.m. on November 16, 2004, Employee #1, a painter, set up a 16-ft aluminum extension ladder by leaning it against the roof of the residence without securing it. Employee #1 climbed up the ladder, carrying a gallon paint container in his right hand. As Employee #1 was about to step onto the roof, the ladder slipped on the concrete ground and he fell 8 ft to the ground. Employee #1 sustained complex a fracture on the right leg and was hospitalized for three days.
