Employee injures leg between pallet jack and guard rail

At approximately 2:55 a.m. on October 25, 2004, Employee #1 was working as an order selector for Ralphs Grocery Company in a distribution center. The main operations in the distribution center were shipping, receiving, and order selection. He was retrieving two pallets of milk from the rear of the warehouse in the dairy section using a pallet jack. As he attempted to make a turn out of the corner, the pallet jack slid and collided with a metal guard rail between two bollards that protect the milk line chain area. Employee #1's left leg became caught between the pallet jack and the guard rail. He suffered a fractured left tibia and was transported by emergency services to the Kaiser Permanente Medical Canter in Bellflower, CA, where he underwent surgery. Further investigation revealed that the floor of the milk line was coated with slip resistant materials and the pallet jack was in good working condition without any deficiencies.
