Investigation Summary
At approximately 3:00 a.m. on January 20, 1999, Employee #1 was operating a forklift truck (Toyota Forklift Truck Number 101, Model Number 6F6CU-25, Serial Number 64365) to transport product from the warehouse to the dock. The surface of the dock was wet from a storm and had areas of accumulated water. As Employee #1 traveled from the warehouse to the slippery surface of the dock, he was seen by another employee to be traveling at higher speeds along the dock's edge. The vehicle began to slide because of the wet surface and proceeded over the edge. It fell down and pinned him to the ground. He sustained fractures to his left shoulder and right lower extremities and was hospitalized for two weeks.
