Employee Amputates Fingers While Using Tire Press

At approximately 2:00 p.m. on September 7, 2006. Employee #1, a tire press operator, was servicing forklift tires at a customer site using the hydraulic tire press mounted on the service truck Number 502. The tire press was set up in the following way: There was a round base plate sitting on top of the ram and a metal cylinder was placed on top of round base plate. The diameter of base plate was 25-in. and the height of the metal cylinder was approximately 1-ft. The old forklift tire, with rim, would be placed on top of the cylinder and a new tire would be placed on top of old tire with the rim. Once the set-up was completed, the press operator moved the lever on the left side of tire press to move up the ram. As the ram goes up, the new tire would come in contact with the stationary top plate and the new tire would be pressed onto the rim, replacing the old tire, which drops onto the metal cylinder below. Once this process was completed, the operator would release the pressure and the ram would return to the starting position by the gravity. Employee #1 serviced the last tire at the customer site and he released the pressure by lowering the lever on the left side. The ram began to descend and there was approximately 6 in. clearance between the new tire and the top stationary plate. Employee #1 reached his hands to remove the new tire from the press. Suddenly there was a loud explosion and the cylinder jumped back up, catching Employee #1's gloved left hand in between the new tire and the top plate. Employee #1 continued to remove the new tire from the press and started to feel the pain in his hand. Employee #1 removed the glove from his hand and realized that his fingers were amputated from the impact. At this point Employee #1 went inside the warehouse and asked the site employees to call emergency services. Employee #1 was treated for his injuries but was not hospitalized.
