Investigation Summary

At about 12:15 p.m. on August 15, 2006, Employee #1, a delivery driver for the Pepsi Bottling Group #1, was making a delivery to a site. When the employee arrived, he was asked to carry the merchandise to the back of the establishment. The employee was carrying 5 crates of product on a dolly, with each crate measuring 8 in. by 11.5 in. by 16 in. As he was taking the product to the back, he lifted a metal counter top and rested it against a wall. He noticed that the counter top was not stable and held it with his left hand to ensure it did not close. He then pivoted around to bring the dolly through the counter with his right hand. He released his left hand from the countertop to remove a stroller that was stored under the counter. At this time, the counter top came down and partially severed his left ring finger. He was transported to the Monterey Park Emergency Room in Monterey, CA, where he was treated for his injury.
