Employee caught in machine after powering it on

At approximately 8:00 p.m. on July 29, 1999, during the second work-shift Employee #1, a maintenance mechanic, was working alone changing the belts on the three Esmach ISE/160 FL spiral mixing machines. Employee #1 was instructed by his immediate supervisor to do maintenance on the mixing machines in a certain department. He had locked-out and tagged-out the first of the three assigned spiral mixing machines and completed the belt change on the first machine. He then took a break. When he came back from lunch, he began working on the second spiral mixing machine. Employee #1 thought that the second spiral mixing machine was locked-out and tagged-out. He took the cover off and began the maintenance procedure. The purpose was to change the belts. He was reaching into the top belt and pulley area when his body pushed up against the on/off button resulting in activation of the spiral mixing machine. His forearm was caught between the six belts and the smaller of the two pulleys but succeeded in releasing his arm from the belt and pulley. Employee #1 received injury to the left wrist which fractured the radius and fifth metacarpal and was hospitalized.
