Employee injured when hand caught in machine

On December 19, 2001, Employee #1 was attempting to diagnose a noise problem with the chain and sprocket unit on a Holland Star Screen (Number 31591-551005) in the sorting area of a recycling operation. He and a coworker stopped the machine and removed the panel covering the chain and sprocket. The coworker was standing at the control panel on one side of the machine and started the unit while Employee #1 observed the chain and sprocket on the other side. Employee #1 lost his balance and his hand became caught in the chain and sprocket. He sustained the amputation of his index and middle fingers and a partial amputation of the ring finger on his left hand, and was hospitalized.
