Investigation Summary
At approximately 8:00 a.m. on September 21, 1998, an employee entered the parking lot in the company van after making a delivery. The employee got out of his vehicle and tried to manually open the metal gate by applying force. The gate wheel came off of the track and fell on the employee. He suffered fractures to various parts of his body and was hospitalized over two weeks.
