Investigation Summary

At approximately 2:45 p.m. on March 1, 2004, an employee, a material applicator leadman, was moving urethane sheets with a rolling stock cart purchased from McMaster Carr, Catalogue Number 2204T13. The rolling cart was a 1,000-lbs-capacity cart that was loaded with 75 urethane sheets. According to the employee's immediate supervisor, each sheet weighed from ten to eleven pounds each, making the maximum total load weight approximately 825 lbs. The employee stated that he had rolled the cart into room T118 to move it away from the roll-up door. As he was moving the cart and load past a stationary parts rack, his thumb tip was pinched between the rolling cart and a corner of the rack. His thumb tip was amputated. The employee went to his supervisor who activated the emergency contact system. The employee was transported to Antelope Valley Hospital with his thumb tip. He was in the hospital for approximately 4 hours and was released to follow up his care with a hand specialist. His thumb tip was not re-attached. Later he had a procedure at the hand specialist to stretch skin over the tip of the injury.
