Employee's' toe amputated in a lift gate mishap
On March 15, 2003, Employee #1 was riding up on a liftgate, and as it rose, he slipped and his foot got pinched between the gate and the truck body. His right toe was amputated and toe's distal bone was fractured. He was hospitalized for the treatment of his injuries.
