Investigation Summary
At 10:59 a.m. on April 2, 2004, Employee #1, a day laborer, was working at a one story, single family dwelling that was being remodeled. As Employee #1 descended an extension ladder that was leaned against the edge of the roof, at about the fourth step from the bottom, he leaned back and the ladder came away from the roof. He continued backwards, stumbling into another ladder, and fell face first in the driveway. Employee #1 was hospitalized and treated for head and facial injuries. He was released three days later.
