Investigation Summary
At approximately 1:45 p.m. on November 11, 2003, Employee #1, the assistant manager of the receiving dock at a retail store, was working with stored inventory. This retail store is located on a particularly small site so that they have placed overseas containers in the rear of the store for the storage of excess merchandise. The containers are stacked 2-high and to access the upper ones they used a Daewoo 4000-lb capacity forklift, with an attached personal platform. The platform had only a single chain at 22 in. for the front guardrail, and it did not have a guard covering the moving parts of the mast. Employee #1 had just ridden in the platform up to one of the containers, and, as he rode back down, he was resting his hand on the unguarded mast. Employee #1's right ring finger and little finger tips were crushed in the mast. Employee #1's ring finger tip was amputated and his little finger required a skin graft. Employee #1 was hospitalized.
