Employee Is Injured in Fall from Frame Ladder

At approximately,12:04 p.m. on September 1, 2006, Employee #1 and a coworker were working on a portable scaffold with the planks at 4 ft high. The employees were using a 6-ft folding ladder to access the work surface. As Employee #1 stepped off the scaffold and onto the ladder, the ladder tipped and Employee #1 fell to the floor striking the back of his head. Employee #1 was taken to the hospital and treated for his injuries.
