Investigation Summary
At approximately 9:00 a.m. on April 27, 2006, Employee #1, a foreman, was working alone. He was operating an Ingersoll Rand Brand DD-22 vibratory compactor. The edge of the road had a trailing edge approximately 12-in. thick at an angle of about 30 degrees and 16-in. wide. He was attempting to cross this incline and the compactor rolled over. He was not wearing his seatbelt. When the machine started to roll over, he was thrown out. His head was caught between the top roll bar and the ground, and he was killed.
