Investigation Summary
On January 31, 2005, Employee #1 and a coworker were installing an advertising banner on the southwest corner of an Aaron's store. They were working from the mid-roof level at a height of approximately 11 feet. They accessed the mid-roof by means of a ladder. After attaching the banner, the coworker descended the ladder to the ground level. Then, as Employee #1 stepped on the first rung to descend the ladder, the ladder slipped and he fell to the ground. He sustained a left radius fracture and numerous facial fractures and was transported to Harbor UCLA Hospital for medical treatment and hospitalization.
