Investigation Summary
On October 29, 2002, Employee #1, an actor, made a parachute jump from 14,000 ft during the filming of a military-themed movie in Oceano Dunes State Park, CA. He landed in the water instead of the beach as planned. A rescue attempt with personal watercraft was unsuccessful, and his body was found three days later. The jump had been practiced on the ground and a practice jump had been successfully executed that morning. A safety conference had been held and an emergency action plan was written, but not followed. The California Department of Parks and Recreation arranged to have a 47-ft U.S. Coast Guard boat available for possible deep water rescue, however the area/jump coordinator ordered this boat to stay about three miles away to remain out of camera sight. Citations were issued for not following the written plan, and for sending the Coast Guard boat away.
