Investigation Summary
At approximately 10:20 a.m. on December 2, 2002, Employee #1was working as a laborer performing general cleanup activities on the project. At the time of the accident Employee #1 was sweeping the floor. There was a piece of plywood on the floor that Employee #1 picked up and began to slide it forward to get it out the way of his sweeping. The plywood was labeled that it was covering a hole but Employee #1 did not read what it said. Employee #1 slid the plywood forward and fell 12 ft through a 28-in.-by-28-in. hole. Employee #1 suffered a fractured right arm and was hospitalized.
