Investigation Summary
On August 20, 1999, Employee #1 was performing maintenance on a ribbon extruding machine. Employee #1 was to remove plastic buildup from a fiber in-running roll. Employee #1 decided to use a small square of paper to clear the buildup. The paper allowed his fingers to enter the point of operation resulting in amputation of two fingertips.
