Employee walks off unguarded scissor lift
On January 2, 1999, Employee #1 was painting theatrical backdrops hanging on a wall using a scissor lift. The safety chain was not fastened and she walked off the end of the lift and fell approximately 8 ft. Employee #1 was hospitalized with a concussion.
