Investigation Summary
Two employees were moving a roll of black cloth on a movie set. Employee #1 was walking backward when his heel and ankle came up against a 4-in. piece of pipe. This caused employee #1 to trip and drop the roll of cloth on his foot. The roll of cloth weighed approximately 140 lb. Because his foot was pinned and could not move, Employee #1 fell backward, snapped his ankle and fractured his ankle.
