Investigation Summary
At 5:30 a.m. on August 25, 2107, an employee was manually pulling out rebar mesh from the welding presses and his coworker thought he heard an "all clear" be sounded. The employee amputated his left index fingertip when the coworker closed the press, catching the employee's fingertip between the presses. Hospitalization was not required.
