Employee's fingers crushed in drive belt

At 9:00 a.m. on April 20, 2002, Employee #1 with American International, was testing the belt tension on a Ringmullor machine (HP: 30, MTR #10) by touching the belts from inside. The drum moved, causing the wheel to turn and crushing the employee's right hand. Employee #1's index finger was amputated and his middle and ring fingers were fractured.
