Employee's Foot Run Over by Front Guide Wheel

At approximately 1:15 p.m. on January 25, 2002, Employee #1, a welder, was installing a 78-inch steel pipe inside a 23,000-foot section of 84-inch concrete pipe. Employee #1 was guiding the last short joint West of the access pit. While backing up the concrete pipe, his left foot caught on the old pipe's grout plug. The operator of the pipe machine moved the new pipe forward, and Employee #1's left foot was run over by the front guide wheel, fracturing his foot. Employee #1 did not realize his left foot was against the grout plug. Employee #1 was hospitalized to treat his foot.
