Employee killed when crushed by truck

Employee #1 and a coworker were unloading boxes from a trailer in a loading dock. After the right side of the trailer had been cleared, the driver of the truck was instructed to pull out of the dock so that the left trailer door could be opened to resume the unloading process. As the truck lurched forward, a box fell from the trailer and landed in the loading pit. Employee #1 jumped down into the loading pit to retrieve the box. He then turned around and put both hands on the loading dock to push himself back up when the truck rolled back, pinning him against the backstop of the loading pit. Employee #1 was crushed by the truck and killed.
