Investigation Summary
On July 9, 2001, at the packing house unloading area of Phil Giba Farms, Employee #1 was cleaning the area around the hopper that the onions were dumped into. He signaled a driver to stop when his trailer was lined up with the hopper. The driver had emptied the first of the two trailers he was hauling, closed the door on the trailer, got into his truck. Checking his mirrors, he did not see anyone. He began to pull forward to unload the second trailer. Employee #1 happened to be between the trailers when the driver pulled forward. Employee #1 was pronounced dead at the scene.
