Investigation Summary

Investigation Nr: 202462941
Event: 06/15/2006
Employee Sustains Fractures in Fall from Scaffold

On June 15, 2006, Employee #1 was measuring the length of the eave on a two-story home. A scaffold was erected and was fully planked. The home was a custom designed with a circular entrance, which made placing the scaffold close to the area somewhat difficult. The distance from the plank to the edge of the building was approximately 2 ft. The employee was walking backwards taking a measurement, when he approached the rotunda area and fell on the inside of the scaffold. He fell approximately 12 to 15 ft. Employee #1 fractured his ribs and clavicle and was hospitalized.
