Investigation Summary
At approximately 3:00 p.m. on August 5, 1999, Employee #1, a maintenance mechanic, was diagnosing a problem with a large commercial mixing machine. During the examination, the mixer started unexpectedly, and Employee #1's finger was pinched between a machine drive belt and pulley. Employee #1 was taken to the hospital where he was treated for multiple injuries to his left index finger and was hospitalized.
