Investigation Summary
At 12:30 a.m. on August 16, 1998, a serious injury accident occurred at the Fantasy Cookie Company. Employee #1 and others were working the midnight shift. The conveyor mechanism jammed and Employee #1, without supervisory approval since there was no supervisor present, chose to attempt the repair. He turned off a power source and climbed under the conveyor. He took hold of a drive chain and pulled. The jam cleared and Employee #1's finger was pulled into the chain sprocket assembly, partially amputating his finger. Employee #1 was hospitalized.
