Investigation Summary
At approximately 11:30 a.m. on October 13, 1998, Employee #1 was working as a roofer and stepped onto a decorative wood beam projecting from the roof eave of a single-story house. The beam broke loose from its nailed position. Employee #1 fell 11 ft 8 in. to a concrete walkway. Employee #1 was transported to the hospital where he was admitted with rib, shoulder, and head injuries.
