EMPLOYEE INJURED WHEN FINGER CAUGHT BY MACHINE

At 8:30 p.m. on April 23, 1998, a serious injury accident occurred inside a production room of the employer's manufacturing company. Employee #1, a machine operator, was attending a calcium tablet fabrication compressing machine, identified as a Stokes 35 Station machine. While attempting to remove broken tablets from the feed frame compartments with a spatula extension tool, Employee #1's left first finger was pinched in a point of operation area of the machine. The machine had not been stopped and de-energized prior to the cleaning operation. Employee #1 was taken by a coworker to the Heartline Medical Clinic in Van Nuys for emergency treatment of a partial finger amputation.
