Investigation Summary
At approximately 10:30 p.m. on April 4, 1998, Employee #1 was restocking the warmer with blankets in the sub sterile operating room. He was exposed to vapors from a canister, with the sterilant concentrate Peroxyacetic Acid. The canister was left in the sink by an employee who used unsafe work practice and procedures in disposing of the canister. Employee #1 was hospitalized.
