Employee injured in fall from rolling scaffold

Marquez Constructors, Inc., was retrofitting a concrete reservoir against earthquake damage and had hired Employee #1 to do manual work. He had been working for two weeks inside a large concrete water tank, approximately 104 ft wide by 202 ft long by 12 ft high. According to Employee #1, at approximately 1:25 p.m. on February 13, 1996, he was standing on a 6 ft rolling scaffold and pulling it sideways toward the next beam to install a bolt when the rolling scaffold hit an obstruction on the floor. The scaffold tipped over, injuring Employee #1. He sustained contusions on his left arm and above his left ear. Employee #1 was transported to Sharp Hospital for treatment and observation. No defects were found with the tools and equipment. An accident-related violation, T8CCR 1664(f)(1), was cited and other violations, 1509(c) and 1637(n)(1), were observed during the inspection. Citations with penalties were issued.
