Investigation Summary
At approximately 8:30 a.m. on December 20, 1995, Employee #1 entered a hoist line while the hoists were running in order to clean an observation window in a large electrolysis copper machine. He was wiping a glass window with his left hand when the hoist pinned his chest against an internal post. He was able to squeeze himself out and lay down on the catwalk. He sustained bruises on his upper chest and back and was taken to Scripps Hospital for treatment and observation. The employer manufactures circuit boards using an automated electrolysis machine, and no malfunction in equipment or procedures was found. Employee #1 was properly trained and had almost one year of job experience. He violated the employer's safety rules and regulations and was issued a reprimand. No accident-related violation was discovered; however, one regulatory violation was observed during the inspection, and the following cited violation was issued: T8CCR 461(a).
