Investigation Summary
At 3:07 p.m. on November 29, 1995, Employee #1 was on top of an old Target Store in La Mesa, CA, that his employer was demolishing and salvaging. He was standing on 4 ft by 8 ft by 3/4 in. plywood roof sheeting, pulling nails. The straps for a vent duct were nailed to the underside of the lumber. Employee #1 had failed to release all the straps; a strap was still holding the duct work approximately 3 to 4 feet from the near end. As soon as the nail was removed from the strap, the north end of the vent duct (18 in. by 24 in. by 40 ft) lifted up, causing the plywood to come out of its hangers and slip and fall. Employee #1 fell 22 ft and landed on piles of roof insulation and debris. Employee #1 was taken to Sharp Hospital, where he underwent surgery for hip and pelvic fractures. No accident-related violations were found; however two regulatory violations were discovered and the employer was cited accordingly.
