Investigation Summary
At about 7:30 p.m. on October 30, 1995, Employee #1, a 43-year-old molding press operator for Accurate Elastomer Products, Inc., was removing brake diaphragms from a Wilcox molding press. The lower half of the top platen momentarily became stuck during the opening cycle, but came down on both of his hands, pinning them between the top and bottom platens. His left index and middle fingers and his right index finger were amputated. Coworkers in the department heard his scream and came to his aid. 911 was called and Employee #1 was taken to a local hospital emergency room. The press had a two-button closing procedure and opened automatically after going through a 7 minute molding cycle. Employee #1 had less than 2 1/2 weeks experience on the machine and was not following operating procedures when he attempted to remove finished products before the lower mold reached the bottom of its cycle. The employer was cited for training deficiency.
