Investigation Summary

On March 2, 1999, Employee #1, a full time union ironworker, was standing on a beam waiting to receive a pick of a single steel truss, from the crane operated by a co-worker. Another employee, also waiting for the truss, received his end first. This caused the truss to swing toward Employee #1, who took a step backward to avoid the truss. Employee #1 lost his footing, jumped to catch himself by swinging down from the truss, and lost his grip on the truss. Employee #1 fell a distance of 12 feet, 6 inches, fracturing his shoulder.
