Employee killed in trench cave-in

At approximately 2:00 p.m. on March 12, 1997, Employee #1 was in a trench, cleaning dirt off the wall of a building prior to installing waterproofing material. The foreman and a coworker were measuring the width of the 8 ft 10 in. deep trench for placing speed shoring when the edge of the trench caved-in, carrying the foreman into the trench and burying Employee #1. The coworker was able to jump clear of the trench. Employee #1 was removed from the trench about 45 minutes later; he had sustained critical injuries, from which he died. The trench did not have any protective system in place. The employer was cited for an accident-related violation of T8CCR 1541.1(a)(1), for lack of a protective system in an excavation.
