Investigation Summary
At 2:00 p.m. on August 5, 2017, Employee #1, a meat cutter, was operating a meat band saw, cutting slabs of bone in short ribs. The employee's right hand slipped off of the meat slab, and three fingers went into blade. Three fingers were amputated, but one was reattached. The employee was hospitalized for his injuries, and reattachment of one finger.
