Investigation Summary

Investigation Nr: 170591770
Event: 04/11/1995
Employee's finger amputated by hydraulic ram

Employee #1 was using a hydraulic pusher to move an empty kiln car up to a setting machine that was mounted in the floor between the tracks. A second kiln car was being repaired on the tracks and was not to be moved. Employee #1 was using an Allen wrench to hold down the dog (the finger that engaged the first car) on the empty car, allowing it to pass underneath the car being repaired. As the dog was about to move under the second car, Employee #1 became distracted and his tool slipped. His right index finger became caught by the dog on the hydraulic ram, and was amputated.
