Investigation Summary
Employee #1 was working below ground at station level on a metro rail project. He was part of a crew assigned to build an elevated wood platform for use in removing ceiling materials. The platform spanned vertical shoring towers and the workers were installing scaffold planks on top of 4 in. by 4 in. posts. Employee #1's coworker had left to go to the bathroom and he asked other workers at the mezzanine level to send up more planks. Employee #1 was continuing to install horizontal planks when one broke and he fell approximately 32 ft to the ground. He was killed.
