Investigation Summary
Employee #1 was attempting to climb to his work location. He was on an I-beam, approximately 6 ft above the ground, and grabbed onto the asphalt atop a 4 ft high deck beam to pull himself over. The asphalt was not adhered to the top of the beam and Employee #1 lost his grip and balance, and fell to the ground. He suffered acute blunt trauma to the back, and back abrasions and contusions. Employee #1 had not been provided a ladder to gain access to his work location. This was his first day on the job.
