Investigation Summary
At 10:30 a.m. on January 22, 2002, a serious injury with multiple fractures occurred at Discover Plastics, Inc. The employer is engaged in the reprocessing of plastics by grinding and extruding the scrap plastic material. The employer had 20 employees and was non-unionized. At the time of accident, Employee #1 was working on a steel structure elevated platform, 75 inches above the ground level, to feed the material into a grinder, a Nelmor Model 2030. Just before the accident, the box of the material, about 3-feet by 3-feet by 3-feet, was placed on the top of two 5-gallon plastic buckets. Employee #1 was also standing on the top of another 5-gallon plastic bucket to reach the material while feeding the material as bundles of plastic hoses, into the grinder. One bundle broke and Employee #1 lost his balance and fell to the concrete floor because the 5-gallon bucket was very close to the unguarded edge of the work platform. Employee #1 received multiple fractures to his right arm and right hip and was taken to Kaiser Permanente Hospital by another employee where he was hospitalized for 10 days.
